Dr. Didier Demesmin and staff would like to welcome you to University Pain Medicine Center. We are pleased that you have chosen University Pain Medicine Center to treat your pain condition. Our goal is to provide you with the most comprehensive and latest interventions available in pain treatment options, offering superior clinical care, up to date minimally invasive procedures, all with the latest technology.

Dr. Demesmin offers a specialized approach to diagnosing and treating pain. An individualized design of care for each patient is made with the mission of improving function, alleviating pain, and enhancing the quality of life.

Our main office address is 33 Clyde Road, Suites 105 & 106 in Somerset, NJ (located at Towne Professional Park at Somerset), and the satellite offices located at 111 Union Valley Road, Suite 202 in Monroe, NJ and at 1810 Park Avenue in South Plainfield, NJ. For the convenience of our patients, all of our offices have ample parking with a handicapped-accessible entrance.

We encourage your active participation in your care. We will discuss your plan of care with you, if you have any questions please don’t hesitate to ask. We want you to be pro-active and write down any questions you have so that when you come in for an office visit we can discuss any concerns you may have.

PLEASE BRING THE FOLLOWING TO YOUR SCHEDULED APPOINTMENT:

We make a commitment to provide you with the best care. Ask about our new patient information packet to be completed at your home so that you can avoid filling the forms at the office. We ask that you bring the following items to your first visit:

• MRI Films and Reports, CT Scan Films and Reports
• EMG Report
• If you have been seen by a Pain Management Specialist, please bring a list of the procedures that have been done or if possible, a copy of the office notes
• Enclosed paperwork should be filled out and brought to the first visit
• Insurance Card / Insurance Information (if your injuries are related to an Motor Vehicle accident or a workman’s compensation case, please bring the insurance information and the adjuster’s name and phone number)
• Valid Photo ID
